What We Offer

The rehabilitation unit offers 24 hour fully qualified care staff, who have undergone rehabilitation training to continually improve person centred outcomes, for each individual to return to a suitable place to live, as independently as possible.

Professional assessments and personal care plans will be completed in collaboration with other relevant professionals, family and representatives, to gain a full assessment of each individual’s needs.

The management team will work closely with the multi-disciplinary team and families to arrange a suitable discharge date and time and this is usually done within a 14 day period.
