Rachel's House Rehabilitation Unit

Rachel’s House is a bespoke build of a 6 bedroom bungalow within the existing grounds of The Dene Lodge care home. The design is to accommodate people who require short term rehabilitation but do not require a hospital setting. All rooms are en-suite, with accessible facilities to accommodate people with physical disabilities. Our location in West Somerset means we are well placed to serve the community hospitals, working in direct partnership with the NHS, community nurses, doctors and physiotherapists.

What we offer

The rehabilitation unit offers 24 hour fully qualified care staff, who have undergone rehabilitation training to continually improve person centred outcomes, for each individual to return to a suitable place to live, as independently as possible.

Professional assessments and personal care plans will be completed in collaboration with other relevant professionals, family and representatives, to gain a full assessment of each individual’s needs.

The management team will work closely with the multi-disciplinary team and families to arrange a suitable discharge date and time and this is usually done within a 14 day period.
